Helicobacter pylori is a spiral-shaped bacterium that infects well over 75% of the world’s population. In some countries it infects 100% of the population. This is, therefore, one of the most common bacterial infections known to mankind. Between 1979-82, Australian pathologist, Robin Warren and Australian gastroenterologist, Barry Marshall, identified H. pylori and suggested a link to the development of stomach ulcers. Since this discovery, the World Health Organisation has declared the bacteria to be a Class 1 carcinogen (meaning the bacterium produces cancer). It invades the mucosal lining of the stomach and is the cause of up to 95% of duodenal and up to 75% gastric ulcers and has also been associated with gastric cancer, acid reflux and lymphoma.
H. pylori infects both genders equally. The presence of H. pylori in the stomach induces a chronic, active, inflammation in almost everyone infected.
Most infections occur in childhood. Crowded living conditions, poor sanitation, poor personal hygiene and a poor water supply correlate with higher rates of infection (which can approach 100% of the population in the developing world).
H. pylori is the only bacterium that has been classified as a Class 1 carcinogen by the World Health Organisation and is found in up to 80% of the world's population.
Stomach Ulcer medications which make use of antibiotics and Proton pump inhibitors are becoming less effective as H. pylori slowly increases its resistance to these treatments. Call your doctor if you experience signs of liver problems: unusual tiredness, persistent nausea or vomiting, severe stomach or abdominal pain, dark urine, yellowing eyes or skin.
Side effects include nausea, diarrhea and abdominal pain. Do not take if you have kidney or liver problems. Other side effects include gray-black stools, vomiting, stomach upset, changes in taste, headache and sun sensitivity.
Usual administration of Prevpac involves swallowing two 500-mg amoxicillin capsules, one 500-mg Biaxin tablet, and one 30-mg Prevacid capsule 2x/day before eating. Side effects include diarrhea, headache and changes in taste.
Symptoms experienced by infected patients prior to taking the above drugs can include no symptoms at all, burning pain in the upper portion of the abdomen, indigestion, nausea, vomiting, burping, acid reflux and loss of appetite.
There are many different tests used to diagnose H. pylori infection. Tests for H. pylori can be divided into two groups: a. invasive, which require upper gastrointestinal endoscopy (gastroscopy) and are based on the analysis of gastric biopsy specimens, and b. non-invasive.
The test has a sensitivity and specificity of 96% for detecting H pylori infection. False-negative results may be obtained on specimens from patients who have ingested selected medications (antimicrobials, proton pump inhibitors, bismuth preparations) within the 2 weeks prior to specimen collection. If clinically indicated, the test may be repeated on a new specimen obtained 2 weeks after stopping treatment with these medications. Positive results are not affected by medication.
A positive result ≥7 days after starting therapy is indicative of ineffective treatment or recurrence. Testing for eradication should be performed ≥4 weeks post therapy; a negative result at this time indicates eradication of the infection.
H. pylori has been strongly linked to the development of gastric and duodenal ulcers. Eradication of H. pylori can prevent stomach ulcers forming. Indeed patients presenting with stomach ulcers should be tested for H. pylori and treated because eradication of H. pylori in patients with pre-existing stomach ulcers cures ulcer disease and can prevent most recurrences.
Gastric adenocarcinoma (Stomach Cancer) is the second leading cause of cancer death worldwide. There is strong evidence that H. pylori contributes to the development of gastric cancer. H. pylori infection is most closely associated with stomach cancer. Hence, due to its known association, all patients with H. pylori should be treated to prevent development of stomach cancer.
H. pylori infection can also lead to the development of a condition known as mucosa-associated-lymphoid-tissue (MALT) Lymphoma, a type of cancer of the stomach. Treatment and eradication of H. pylori infection can result in regression of the malignancy in up to 75% of cases.
Treatment for H. pylori focuses on eradicating the bacteria from the stomach using a combination of organism-specific antibiotics with an acid suppressor and/or stomach protector. The use of only one or two medications to treat H. pylori is not recommended. Different countries have different approved treatments for H. pylori. This regimen of triple therapy reduces stomach ulcer symptoms, kills H. pylori and prevents stomach ulcer recurrence in around 70% of patients but its efficacy is slowly falling as the H. pylori bacteria becomes resistant to the antibiotics.
With the use of antibiotics to treat so many patients with various conditions it has become more difficult to treat H. pylori due to increasing occurrence of antibiotic resistant strains. As a result, up to 35% of patients fail the first line therapy.
In those patients who have been treated for H. pylori and the bacteria continues to be present, for example as determined by a Urea Breath Test, a further treatment using the previous therapy should not be tried again.
Unfortunately once a patient has been treated using a standard first-line treatment, but fails to eradicate the infection, the H. pylori remaining can become resistant to a number of the antibiotics used. As such it is imperative that the patient is treated with different antibiotics to reduce the likelihood of creating a "super-resistant" infection.
Second-line treatments, which are adjusted to suit each patient based on the number and type of previous treatment, can include the following medications: Rifabutin, Bismuth subsalicylate Furazolidone, Lactoferrin, Nitazoxanide and Levofloxacin.
